Find 2008 Summer Grant information under 2007-08 academic year forms. Use 2008-09 academic year forms for Fall 2008 or Winter/Spring 2009 enrollment. Refer to submission deadlines found on each form.

Summer State Grant Application: Use if you will attend 2008 summer classes. The deadline for application submission is August 15, 2008. Although you may still apply after August 15th, your application will be considered late and may not be approved, unless you are attending an approved business, trade, or technical school. Incomplete Grant Status Notice: Use to supply additional information necessary for completing your State Grant record. You do not need to fill out the entire form - only submit the information that is incomplete on your record.

Reduced Income: Use if you or your spouse experience a reduction of income due to: (1) a recent job loss or change; (2) retirement; (3) permanent and total disability, or (4) upon the recent death of a spouse. Download 2008-09 Form (140 KB PDF)
Income Validation: Use to provide PHEAA with a complete copy of your (and your spouse's, if married) U.S. Income Tax Return(s), all supporting wage and tax statements (W-2 Forms) and all supporting schedules and tax forms. Download 2008-09 Form (581 KB PDF)
Estimated Income Validation: Use to verify your income if you submitted your FAFSA using estimated family income because you hadn't yet filed your tax return with the IRS. Recent Separation/Divorce: Use if you experience a recent marital separation (of at least two full months) or a divorce. Download 2008-09 Form (133 KB PDF)
Medical/Dental Expense: Use if you or your spouse paid extraordinary out-of-pocket unreimbursed medical or dental expenses. Download 2008-09 Form (109 KB PDF)
Family Size/Number Enrolled Clarification: Use to make corrections/updates to the number of household members that: (1) will receive more than 50% of your support, and/or (2) are enrolled in college for at least six credits. Download 2008-09 Form (114 KB PDF)
October Verification Form: Use to provide PHEAA with an update to your (and your spouse's, if married) 2008 income estimate if you previously advised the agency that you or your spouse experienced a reduction of income due to: (1) a recent job loss or change, or (2) retirement.
